Billing
Policy
Our
charges are standard fees relating to
the type of procedure or examination
performed. Our office participates with
several insurance plans. We will bill
these insurances directly. You will
be responsible for any services that
are not covered by your plan and also
any amounts that are applied to your
deductible or co-insurance.
If
our office does not have a contract
with your insurance company, as a courtesy
we will submit your bill to your insurance
company if you provide us with the necessary
information to do so. However, the responsibility
for the payment rests with you, regardless
of your insurance coverage or status
of your insurance claim. Payment in
full is expected at the time of the
service, unless other arrangements are
made. This helps keep our billing costs
to a minimum and we, in turn, can pass
these savings on to you by keeping our
fees down.
As
required by PA state law, we file all
Workers' Compensation claims on behalf
of the patient. If your visit is work-related,
please provide us with your employer's
name, the worker's comp insurance name
and address. If a worker's comp claim
number has been established, please
also provide this at time of your first
appointment. You will also be asked
to provide your own health insurance
information, in the event of a possible
claim denial by Workers' Compensation.
Our
practice accepts cash, check, Visa,
Mastercard, and Discover. You may contact
our billing department at 741-4666,
and select the option for billing, with
any questions or concerns.

Use of Outside Laboratories
In addition to the services we perform in our office, it is sometimes necessary to send your specimen to an outside laboratory. Our office primarily uses the following outside labs:

You may receive a bill directly from the laboratory depending on your insurance coverage. Any questions concerning their bill can be referred directly to them.
